Becoming a Patient
We currently participate with the following health insurance plans:
Anthem – Blue Cross, Aetna, Cigna, Harvard Pilgrim,
Maine Medical Center – PHO, Med Net, Champus/ Tricare.
If Dr. Robbins is a participating provider with your insurance plan,
then we will go through insurance.
If you have an indemnity plan, or if there is an uncovered portion (deductible,
co-insurance), then after care is rendered the out-of-pocket portion would be
charged by credit card.
If Dr. Robbins is not a participating provider for your insurance plan, then we would wait till after care is rendered and then the surgical procedure would be charged by credit card.
Dr. Robbins will reply to your e-mail.
If you are interested in proceeding, then please send records and Dr. Robbins will review the records and a phone appointment will be set up to go over your history, answer your questions, and make recommendations.
There will be a $150.00 charge for the phone consultation that will be charged by credit card.
If you are interested in becoming a patient, please send the following:
1. A complete copy of medical records, including
- Office visits
- Operative reports
- Pathology reports
- Ultrasound, and other radiology reports (Please do not send films.)
- Medications
- Surgical photographs (not photocopies; I will return originals)
2. It would be helpful for us to know:
- All previous pelvic surgery you have had - date, hospital, procedure?
- Have you taken any of the following medications?
- Lupron, Danocrine, Birth control pills, Depo-Provera, Pain-relief medications?
3. A summary of the current problems you are having:
- Is there pain with sexual intercourse?
- Is there pain with menstrual periods?
- Is there pain the rest of the month?
- Does your pain radiate to the back or down the legs?
- Is there is pain with urination or with bowel movements?
- Are there changes in bowel movements associated with the menstrual periods?
Please do not send us the following: CT, MRI, or Ultrasound films.
Dr. Robbins will review your records, determine whether being treated at our center is likely to be beneficial and Dr. Robbins will be in contact with you to review your history and to go over recommendations for treatment. All surgical approaches are tailored to meet the specific needs and concerns of the patient.
